Player commitment and fee agreement for 2012
Dear Player and Parents, Please accept this letter as your invitation to be a member of Ohio Express Girls Basketball Club. This invitation is for the Spring 2012 season. Please read the following thoroughly. In accepting this offer, the undersigned player will be committed to Ohio Express for the Spring 2012 season. We understand in signing this agreement for the time period listed above that we are agreeing to accept the rules and regulations of the Ohio Express Girls Basketball Club and the below described guidelines and financial requirements concerning payment of all player fees and expenses. This signed commitment binds undersigned player to the Ohio Express for the time period listed above. Player may play for another team ONLY IF the Ohio Express releases the player from this commitment and the player otherwise complies with applicable AAU rules with respect to changing teams. AAU rules state that player must sit out sixty days from the time she is released before she can play with another AAU team. We understand that absolutely no playing or guest playing for other clubs/teams during this period will be permitted. We understand that Ohio Express teams will not be secondary to any other sport. The undersigned player may play another select sport only with the understanding that her participation with the Ohio Express will be her first priority. We understand that all team practices and games are mandatory and all players must attend and be on time. As the parent/legal guardian for the undersigned player, I agree to pay the 2012 player fee of $400 by the deadlines set by the Ohio Express organization. I fully understand and agree to abide by the following financial policy: If player fees for undersigned player are not paid when due, the undersigned player will be placed on probation and not allowed to continue with Ohio Express until such fees are paid. Absolutely no refunds will be given if the undersigned player chooses to leave the Ohio Express after the first payment has been made. The undersigned parent/legal guardian understands that we are responsible for obtaining an AAU card for the 2012 season at the cost of (TBD for 2012) for the undersigned player. We understand that the undersigned player cannot participate in any club activities until an AAU Card is on file. Instructions for obtaining a card are available at http://aausports.org/default.asp?a=pg_membership.htm. After the AAU Card has been obtained, player and parent need to sign and submit to the Ohio Express.
