If you are interested in playing for the Ohio Express in 2010, you will need the following items:

2. Waiver Form- Click here for waiver form
3. Commitment Letter- Click here for commitment letter
4. Copy of your birth certificate
5. $100 Uniform Deposit (check made out to the Ohio Express)

If you are interested in coaching for the Express, you need the following items:
1. Coaches Application- Click here for application
2. Background Check may be requested, if you already had one, please submit it with your application.
